IMPORTANT HEALTH COVERAGE TAX DOCUMENTS
In accordance with the Paperwork Burden Reduction Act, MMUUSD will not automatically mail IRS Form 1095-C.
Digital copies of Form 1095-C will be available through the Employee Access Center (EAC).
To access your form, log into EAC and navigate to Salary & Benefits, then select Tax Forms, and choose Print 1095s.

Requested forms will be provided within 30 days of the request.
